A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social well being.

Social Prescribing Link Workers

We have a Social Prescribing Link Worker service based here at Swallownest Health Centre The Link Worker role can exclusively offer short term support to help patients on a one to one basis to connect to local groups and activities, information and guidance, which will help support their health and wellbeing.

This may include support and information around social opportunities, benefits and finance, healthy activity, employment, housing, education and training, mental wellbeing and self-management of symptoms.

Link Workers (Primary Care Networks)

Clinical Pharmacist

Clinical Pharmacists work as part of the surgery team in many different ways depending on the need of the practice. Clinical Pharmacists help GPs to perform annual structured medication reviews to make sure your medicines are safe and effective. This can involve helping patients with long-term conditions, such as asthma, type 2 diabetes or high blood pressure. The review involves making sure the medicines all work well together and ensuring the relevant health checks, such as blood tests, are performed. You will have a consultation with a Clinical Pharmacist in the same way as you do with your GP or Nurse. A Clinical Pharmacist may be able to prescribe medicines in the same way as your Doctor. If you see or speak to a Clinical Pharmacist, you can still see or speak to a GP if you need to. Other roles can include hospital medication changes, dealing with out of stock issues, supporting care home patients, updating protocols and ensuring prescribing guidelines are implemented.
