Clinical Pharmacists work as part of the surgery team in many different ways depending on the need of the practice. Clinical Pharmacists help GPs to perform annual structured medication reviews to make sure your medicines are safe and effective. This can involve helping patients with long-term conditions, such as asthma, type 2 diabetes or high blood pressure. The review involves making sure the medicines all work well together and ensuring the relevant health checks, such as blood tests, are performed. You will have a consultation with a Clinical Pharmacist in the same way as you do with your GP or Nurse. A Clinical Pharmacist may be able to prescribe medicines in the same way as your Doctor. If you see or speak to a Clinical Pharmacist, you can still see or speak to a GP if you need to. Other roles can include hospital medication changes, dealing with out of stock issues, supporting care home patients, updating protocols and ensuring prescribing guidelines are implemented.

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social well being.

About Mental Health Practitioners
The Primary Care Network Mental Health Team (PCN MH) is part of Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH). This service provides local specialist mental health support for adults aged over 18yrs. in Rotherham. It also supports GPs in the treatment of mental health issues in primary care. This service has several different pathways.

- To enable you to receive help in a familiar setting
- To help deliver a more joined-up approach to your care
- To offer treatment and support based on all of your needs
How they support you
Mental health practitioners aim to promote a person-centred approach to physical and mental health. Your social care needs are also considered as part of this process. The service carries out assessments of your mental health either face to face or via the telephone. Outcomes may include, but are not limited to:

- Signposting and/ or onward referrals to other services or organisations
- Advice on treatment
- Brief interventions to help you manage mental health challenges.
Mental health practitioners will support you to focus on your goals and to access your local community resources. They want to empower you on your recovery journey and as such if it is appropriate for you to self-refer to a service that they think could be helpful to you, they will fully encourage you to take this step.

Paramedic practitioners are trained to independently provide care that does not require the intervention of a doctor. They report directly to the GP with the outcome of their visits and any updates on any treatment and medication that was given. The paramedics also work at the hubs during the weekends, triaging and seeing patients who did not require a GP to attend.
How can the Paremedic Practioner help you?
They can help by:
- attending home visits on behalf of your GP practice.
- assessing and treating health conditions.
- ordering tests and interpreting results.
About Practice Paramedic Practitioners
These are experienced practitioners who have previously had a lot of experience with an ambulance service and gained additional skills and experience working directly with GP practices. They help the practice with visits and offer appointments for certain problems.
How they support you
They may visit you at home or you may have an appointment or review with them at your GP surgery.
What can you expect
A full, comprehensive assessment either at your home if needed or maybe the GP practice. They will diagnose, treat, and link in with other members of the practice team or other services as needed.
About Physiotherapists
Our advanced PCN physiotherapists work directly with the practice and PCN to provide assessment and treatment for a range of conditions that can affect your functioning and mobility. They can diagnose and treat a wide range of conditions and are the right person to see first.
How they support you
Our experienced physiotherapist will assess you and offer support, treatment and referral if needed.
What can you expect
You may have a telephone call or a face-to-face assessment with our physiotherapist after calling the practice. They will help and guide you about advice and you may need a follow up with them. They can prescribe and refer you to a more formal course of physiotherapy for more investigations if needed.

Each practice has a dedicated social prescriber who can take a holistic, complete view of our patient and help come up with options and solutions that will help. They link in with many support services that are available including mental health support, finance support and advice, community support groups and lifestyle advice.

What can you expect
You can access our social prescribing team via reception direct or you can be referred via anyone in your practice. A social prescriber will arrange to assess, with your permission and they will try and help you put together a plan to help your wellbeing.
We have a Social Prescribing Link Worker service based here at Swallownest Health Centre The Link Worker role can exclusively offer short term support to help patients on a one to one basis to connect to local groups and activities, information and guidance, which will help support their health and wellbeing.
This may include support and information around social opportunities, benefits and finance, healthy activity, employment, housing, education and training, mental wellbeing and self-management of symptoms.
